Can you try changing your Wine version to tkg-4.5?
And if that doesn’t help, try experimenting with graphical options and maybe repairing the game.
Reinstalling the game using our installer might help too.

Are you on Wine StagingHQ 4.5? Did you Disable Lutris Runtime and enable Prefer System libraries in your system options?

It looks like setting the same overrides and environment variables as the latest script installer, as well as setting graphics to low, works. After some more experimentation, this seems related to the anti-aliasing setting.
